Manifest Request
| To obtain Illinois Uniform Hazardous Waste Manifests,
which are required for all Special Waste (hazardous and non-hazardous)
generated in Illinois or destined for Illinois, follow the instructions
below. The cost is $3.00 per manifest for Hazardous Waste Manifest and
$4.00 per manifest for PIMW Manifest.
Online Purchase - The online purchase process is temporarily
